Submission Details

Submissions must follow the format below. Maximum limit: three pages.

Please provide the following information:

1. Nominee/Applicant: An individual or organization that owns or is a partner in the implementation of the project/initiative. Please provide:
   • project or initiative title;
   • sponsoring individuals or organizations;
   • name and title of contact person;
   • contact information, including:
     • mailing address;
     • telephone and fax numbers;
     • email address.
2. Description: Include title, objectives, outstanding attributes and total cost. Also include a photograph of the project or initiative.
3. Demonstrate the direct involvement of Aboriginal people including:
   • individual contributions;
   • organizations;
   • partnerships
4. Give examples of how a positive environmental impact was achieved and how it was measured.
5. Describe how many people are affected by the activity.
6. Describe the educational component, teaching materials and techniques.
7. Illustrate any significant challenges faced and how they were overcome.
